By filling out this registration form for Staff-Logic you certify that the information contained in this application/form is true and correct to the best of your knowledge and you  agree to have any information checked by us. Also, you realize that this information is for the purpose of obtaining
assistance in securing permanent, temporary, or contract employment. You understand that you will never be charged a fee by
Staff-Logic, you
acknowledge the use of this application/form, and by filling it out, does not indicate that any positions are open, nor does it obligate
Staff-Logic, to further process your application.
Again, you certify as to the accuracy of the matters set forth herein and in any resume and understand that any mistreatment of facts may cause you
to be refused employment or to lose your employment.  We assure you that your present employer will not be contacted without your prior knowledge and consent. Please be advised that you have the right to request a copy of this.

References:
You hereby authorize Staff-Logic and it's agents to make such investigations and inquiries into your employment and education history and other related matters as may be necessary in arriving at an employment decision. You hereby release employers, schools and other persons from liability in responding to inquiries connected with your application and you specifically authorize the release of information by any schools, businesses,
individuals, services or other entities listed by you in this form.
Furthermore, you authorize
Staff-Logic and it's agents to release any reference information to clients who may request such information for
purposes of evaluating your credentials and qualifications.

Temporary/Contract Employment:
If employed as a temporary or contract employee, you understand that you will be an employee of Staff-Logic and not of any client. You further
understand that your employment is not guaranteed for any specific time and may be terminated at any time for any reason. You further understand that a contract will exist between
Staff-Logic and each client to whom you may be assigned which will require the client to pay the fee to Staff-Logic, in the event you accept direct employment with the client.

You agree to notify
Staff-Logic immediately should you be offered direct employment by a client (or by a referral of the client to any subsidiary or
affiliated company), either for a permanent, temporary (including assignments through another agency), or consulting
positions during your assignment or after your assignment has ended, and this also includes the same for a period of twelve months from end date or termination of any such employment or temporary /contract assignments.

Staff-Logic  or its agents are not a professional accounting firm. You will not render as opinion on behalf of Staff-Logic on financial information
(i.e. financial statements, tax returns, etc.) You will not sign the name of
Staff-Logic or your name to financial information or tax returns for any
client. You will not handle cash, negotiable, or other valuables of any client without the prior written consent of your employer,
Staff-Logic .Under no circumstances will you transport cash, negotiable or other valuables.
